Insurance cover is provided as part of player registration fees. Insurance claims must be completed and submitted within 30 days of the incident.

 To make a claim:          

1. Download and print the Claims form.
2. Complete the sections on page 1 and page 2.
3. Have a dentist, doctor or surgeon complete pages 3 and 4.
4. Post all of the claim forms to:

Northmead NSW 2152       

Adam will then complete the Club Official’s declaration, and submit the form to the insurer.

Please do not wait for medical accounts as these can be submitted later. It is essential that the claim form is completed and submitted to the insurer within 30 days of the incident.
